Magnetic resonance imaging (MRI) of the head/brain is a painless, noninvasive test that produces detailed images of your brain and brain stem. An brain MRI scan combines images to create a 3-D picture of your internal structures, so it’s more effective than other scans at detecting abnormalities in small structures of the brain such as the pituitary gland and brain stem.

Your doctor may recommend either magnetic resonance imaging (MRI), magnetic resonance angiography (MRA) or for magnetic resonance venography (MRV) of your head. All of these procedures use a magnetic field, radio waves and a computer to create detailed images, which is seen by a team of specialist physicians who are used to distinguish normal, healthy tissue from diseased tissue. The MRA and MRV are specifically designed to examine your blood vessels.

Sometimes a contrast agent, or dye, can be given through an intravenous (IV) line to better visualize certain structures or abnormalities. Contrast dye is actually a paramagnetic liquid that responds to the machine’s magnet and helps enhance certain abnormalities on the scan that would not have otherwise been visible, common in neurological MRIs.

The different types of MRI are Open MRI,1.5 Tesla and 3 Tesla MRI. Depending on patients condition, a docotor specifies which MRI is to be conducted.

The doctor can advise a brain or head MRI if you have a symptom may be due to a brain issue like:-

  • dizziness
  • weakness
  • seizures
  • changes in thinking or behavior
  • blurry vision
  • chronic headaches

A head MRI can help determine whether you sustained any damage from a stroke or head injury. MRI can also help to identify the specific location of a functional center of the brain (the specific part of the brain controlling a function, such as speech or memory) to assist in treatment of a condition of the brain. A head MRI /MRA / MRV is a useful tool for detecting a number of other brain conditions too like:

  • traumatic brain injury
  • stroke
  • aneurysms or bulging in the blood vessels of the brain
  • spinal cord injuries
  • brain tumours
  • hydrocephalus – a buildup of spinal fluid in the brain cavities
  • infections
  • cysts
  • inflammation
  • hormonal disorders – acromegaly and Cushing’s syndrome
  • hemorrhage or bleeding into the brain or spinal cord
  • Subdural hematoma (an area of bleeding just under the dura mater or covering of the brain)
  • Abscess
  • Venous malformations
  • Degenerative diseases, multiple sclerosis, hypoxic encephalopathy (dysfunction of the brain due to a lack of oxygen), or encephalomyelitis (inflammation or infection of the brain and/or spinal cord)
  • Congenital abnormalities-problems with development or structure malformation
  • dementia
  • an issue due to a previous head injury

Before your MRI test, tell your doctor and the MRI technologist if you:

  • Are allergic to any medicines.
  • Are or might be pregnant.
  • Have any metal implanted in your body.
    • Heart and blood vessel devices such as a coronary artery stent, pacemaker, ICD (implantable cardioverter-defibrillator), or metal heart valve.
    • Metal pins, clips, or metal parts in your body, including artificial limbs and dental work or braces.
    • Any other implanted medical device, such as a medicine infusion pump or a cochlear implant.
    • Cosmetic metal implants, such as in your ears, or tattooed eyeliner.
  • Had recent surgery on a blood vessel.
  • Are Claustrophobic
  • Have any other health conditions such as hypertension, kidney problems etc
  • Wear any medicine patches

The medical staff will need to know if you have any metal in your body, including:

  • inner ear implants
  • artificial joints
  • a defibrillator or pacemaker
  • particular types of heart valves
  • vascular stents
  • brain aneurysm clips

MRI Machine

MRI Procedure: You will lie down very still on a table that slides into the MRI machine which slides through a large magnet shaped like a tube with a plastic coil placed around your head. After the table slides into the machine, a technician will take several pictures of your brain, each of which will take a few minutes. The test normally takes 30 to 60 minutes. You may receive a contrast solution, usually gadolinium, through an IV to allow the MRI machine to see certain parts of your brain more easily, particularly your blood vessels.

The MRI scanner will make loud banging noises during the procedure. You may be offered earplugs to block the MRI machine’s noises or you may listen to music during the test. There will be a microphone in the machine that allows you to communicate with staff. A radiologist will analyze your MRI images and provide your doctor with the report.

There are no risks associated with an MRI itself. There is a very slight chance that you will have an allergic reaction to a contrast solution.

Brain MRIs are non-invasive and do not use radiation. Typical costs for a head MRI ranges from Rs xxxxxxx to Rs xxxxxx depending on complexity of the condition being monitored, use of contrast material, radiologist fee, whether the procedure is performed in a hospital or a stand-alone diagnostic centre and the area the city. Machines come in a variety of sizes and powers. Their imaging power is measured in magnetic field strength units called Teslas; low-field or open MRI machines measure 0.5, while the strongest currently on the market are 3 Teslas.

The central nervous system (CNS) consists of the brain and spinal cord. The brain is an important organ that controls thought, memory, emotion, touch, motor skills, vision, respirations, temperature, hunger, and every other process that regulates our body. MRI may be used to examine the brain and/or spinal cord for injuries or the presence of structural abnormalities or certain other conditions. The brain can be divided into the cerebrum, brainstem, and cerebellum:

  • Cerebrum – The cerebrum (supratentorial or front of brain) is composed of the right and left hemispheres. Functions of the cerebrum include: initiation of movement, coordination of movement, body temperature, touch, vision, hearing, judgment, reasoning, problem solving, emotions, and learning.
  • Brainstem – The brainstem (midline or middle of brain) includes the midbrain, the pons, and the medulla. Functions of this area include: movement of the eyes and mouth, relaying sensory messages (such as hot, pain, and loud), hunger, respirations, consciousness, cardiac function, body temperature, involuntary muscle movements, sneezing, coughing, vomiting, and swallowing.
  • Cerebellum – The cerebellum (infratentorial or back of brain) is located at the back of the head. Its function is to coordinate voluntary muscle movements and to maintain posture, balance, and equilibrium.
  • Spinal cord – A large bundle of nerve fibers located in the back that extends from the base of the brain to the lower back, the spinal cord carries messages to and from the brain and controls many reflexes.

Magnetic Resonance Venography (MRV) and Magnetic Resonance Angiography (MRA) – MRV and MRA examinations are similar to an MRI, but they focus exclusively on the venous and arterial blood vessels in the head and neck area. A strong magnetic field is used to evaluate blood flow patterns and blood vessel abnormalities.

Magnetic Resonance Angiography MRA—a type of MRI study that is obtained with the same scanning machine using a strong magnetic field combined with radiofrequency waves to create highly detailed images of blood vessels in the body. MRA tests are used to produce two- or three-dimensional images of the structures inside your body, including your blood vessels.

Magnetic Resonance Venography MRV – MRI in conjunction with MRV is both sensitive and specific enough to provide the best non-invasive method of diagnosing cerebral venous thrombosis. MR venography demonstrates absence of flow in certain areas.

MR cisternography – MR cisternography helps in the diagnosis of cerebrospinal fluid rhinorrhoea with diagnostic nasal endoscopy and surgical correlation. It is a noninvasive technique, which does not involve the use of contrast material or spinal puncture.

Magnetic resonance spectroscopy (MRS) – Spectroscopy is used to study metabolic changes in brain tumours, strokes, seizure disorders, Alzheimer’s disease, depression and other diseases affecting the brain.

Perfusion and diffusion magnetic resonance imaging (MRI) are commonly used by neuro-radiologists in everyday clinical practice. These techniques provide information not available with conventional MRI.

Perfusion MRI is a promising tool in assessing stroke, brain tumours and patients with neuro degenerative diseases. The main role of perfusion imaging is in evaluation of ischaemic conditions (e.g. acute cerebral infarction and neuro degenerative diseases.

Diffusion-weighted (DW) MRI is done to visualize internal physiology providing potentially unique information on the viability of brain tissue which may be substantially altered by disease. It is particularly sensitive for detection of acute ischemic stroke and differentiation of acute stroke from other processes that manifest with sudden neurologic deficits. Diffusion-weighted MR imaging also provides adjunctive information for other cerebral diseases including neoplasms, intracranial infections, traumatic brain injury, and demyelinating processes.

Functional MRI (fMRI) – A functional MRI (fMRI) of the brain is useful for people who might have to undergo brain surgery. An fMRI can pinpoint areas of the brain responsible for speech and language, and body movement. It does this by measuring metabolic changes that take place in your brain when you perform certain tasks.
